In accordance with State of Texas guidelines and as an effort to equip you with knowledge about accessing records and reporting issues, the information below is provided.
Record Request
In the absence of applicable state and federal laws, rules and regulations, records and test data shall be maintained for a minimum of 7 years after the date of termination of services with the patient, client, or subject of evaluation, or 5 years after a patient or subject of evaluation reaches the age of majority, whichever is greater.
